Medical Membership

You and your family can become members of our clinics by paying a monthly fee. This membership entitles you to receive medical care for a discounted global fee of $5 per visit. Each visit includes any procedures or treatments without extra charges if they are performed in our clinics. You can join directly and receive Individual rate for $49/month, Family rate available for 5 or more members only or help your company set up a corporate account to receive the Corporate rate for $30/ month. A minimum of 10 people are required to qualify for corporate account.

• Combine this Membership with your high deductible health plan and you will not hesitate to seek medical care to keep you healthy and away from ERs and hospitals.
• If you cannot afford a health insurance, at least you can receive basic medical care at affordable price through this Membership program.
• If you are an Employer, you can offer this to your employee as a health benefit.
• If you are an Employee, you can ask your Employer to set up a Corporate account so you can receive pre-taxed Membership rate of $30/ month.

Services Included:

• Evaluation and treatment for Urgent Care and Primary Care
• All labs performed in the clinics, such as: Rapid Strep Test, Urinalysis, Urine Microscopic Exam, pregnancy and Mono Test. (Sent out labs are not included but offered at discounted prices.)
• All treatment given in the clinics, such as: Nebulizer Treatment, IM and IV Medications, IV Fluid Therapy, Splinting, and Casting.
• All procedures performed in the clinics such as: Laceration Repair, Draining Abscess, Foreign Body Removal and X-Ray

• Membership includes services performed only in 5 Minute clinic.
• Membership does not provide Chronic Pain Management, Alcohol or Substance Addiction and Withdrawal Treatment, and Chronic Conditions Requiring Controlled Medications such as Narcotics, Ritalin, Xanax, Suboxone, and Methadone.
• A 10-Day waiting period applies for Individual Membership.
• There is a commitment of 12 month for Individual Plan with automatic monthly renewal thereafter.
• eFit Financial, a business associate, will host members’ accounts and perform monthly charges and collection.
